All information obtained for this purpose will remain confidential. One form per student enrolled is required.

The Texas Education Agency is mandated by Texas State Law to collect data on military connected students
enrolled in Texas public schools.

Section 25 006, Texas Education Code. The agency shall collect data each year from school districts and open enrollment charter schools through the Public Education Information Management System (PEIMS) relating to the enrollment of military-connected students.

Falsification of Information Texas Education Code 25.001 (h) and (i) Texas Penal Code 37.10

A person who knowingly falsifies information on a form required for enrollment of a student in a school district is liable for the greater of the maximum tuition fee or the amount the district has budgeted for each student as maintenance and operating expenses if the student is not eligible for enrollment in the district but is enrolled on the basis of false information.

NOTE: Enrollment in specific magnet programs does not incur out of district tuition fees. Check with your individual counseling office if your student is enrolled in a magnet program.

DAILY TUITION RATES ARE SUBJECT TO CHANGE. TUITION RATES ARE REVISED YEARLY IN OCTOBER AND ANNOUNCED BY THE TEXAS EDUCATION AGENCY. STUDENTS WHO QUALIFY FOR SPECIAL EDUCATION SERVICES DIFFER FROM GENERAL EDUCATION STUDENTS DUE TO THE ADDITIONAL SERVICES THEY RECEIVE. CHECK WITH YOUR CAMPUS COUNSELING OFFICE FOR CURRENT TUITION RATES.

Proof of Identity of Person Enrolling Student

Regardless of whether or not a child's parent, guardian, or other person with legal control of the child under a court order is enrolling a child, Texas Education Code as amended in 2001, a district is required to record the name, address, and date of birth, of the person enrolling a child. TEC Section 25.002(f). Providing a copy of your government issued ID with photo satisfies this request.

DISTRICT maintains general education records which are available to the parent, guardian, or person standing in lawful control of the student under a court order. Both parents/guardians have access to the records unless the school is in possession of a court order limiting access.

Federal law provides that students "educational records" are confidential. School records are defined as being directly related to a student and maintained by the school including, but not limited to: attendance, grades, discipline, test scores, health and immunization, and psychological or counseling records. Directory information is not confidential under FERPA.

Certain information about district students is considered directory information and will be released to anyone who follows the procedures for requesting the information unless the parent or guardian objects to the release of the directory information about the student. If you do not want DISTRICT to disclose directory information from your child's education records without your prior written consent, you must notify the district in writing by the tenth (10th) day of the school year.

DISTRICT has designated the following information as directory information: student's name, address, telephone listing, electronic mail address, photograph, date and place of birth, major field of study, degrees, honors and awards received, dates of attendance, grade level, most recent educational institution attended, participation in officially recognized activities and sports, and weight and height of members of athletic teams. Student directory information is available to the public unless the parent/guardian restricts the release of the information.

1. According to the Texas Public Information Act (TPIA), DISTRICT must release directory information promptly upon request and may not ask requestors the reason for the requested information
   
   
2. Parents/guardians have the right to restrict the release of directory information, but must do so in writing within the first ten (10) days of the school year or enrollment or see number 4 below.
   
   
3. Parents/guardians may alter their choice regarding directory information at any time by completing and submitting the Student Directory Release form found in the list on the left of this webpage to their child's school. Decisions regarding directory information will remain until a subsequent written request is made to alter the previous decision.
   
   
4. Parents with a Parent Portal account may review their child's privacy status and make changes if wanted. Privacy codes may also be changed any time by completing a Student Directory Information Release form (found in the list on the left of this webpage) and submitting it to their child's school.
   
   
5. Schools must notify parents and eligible students of their FERPA rights annually. In addition to this form, FERPA information is found in the Student/Parent Handbook available in both English and Spanish online at the district website or a paper copy, by request, from your child's school.

During the 89th Texas Legislative Session, lawmakers passed Senate Bill 12, which requires schools to get annual parental consent before providing health-related or counseling-related services to students. This includes things like first aid, short-term counseling, or vision and hearing screenings.

Examples of Health Related Services and Counseling Services That May Be Provided, but are not Limited to:

Health Related Services

• First aid and injury evaluation
• Check temperature, blood pressure, oxygen saturation, and pulse
• Administration of over-the-counter medications per school policy
• Monitoring of chronic health conditions (e.g., asthma, diabetes)
• Vision, hearing, and Scoliosis screenings
• Administer an opioid antagonist, such as Narcan, to an individual who is reasonably believed to be experiencing an opioid related drug overdose on campus
• Support during illness or physical symptoms at school

Counseling Related Services

• General guidance and academic counseling
• Short-term individual counseling or check-ins
• Social-emotional skill-building or group counseling
• Support during emotional distress or crisis, including assessment following a reported concern of student self-harm or harm to others
• Behavioral observations and problem-solving

These services do not replace seeking medical or mental health services from a trained licensed professional.

The purpose of this form is to obtain records so that we can determine appropriate placement and eligibility for your student. We will request records from your student's previous school that may include, but are not limited to:

- birth certificate
- immunization records
- grade reports
- standardized test scores
- special program information such as special education
- 504
- CTE
- bilingual records
- withdrawal form
- discipline records.

I represent that I am a person who has the right to consent to medical, dental, psychological, and surgical treatment on behalf of the identified student.

AUTHORIZATION:
I authorize the school to contact the person(s) identified as emergency contact(s). In the event that the student's parent(s), legal guardian(s), emergency contact(s) and/or nonparent adult caregiver(s) authorized by Texas Family Code Chapter 34 cannot be immediately contacted by telephone, I authorize the school to consent to medical, dental, psychological, and surgical treatment on behalf of the student.

NOTE: In the event the school consents to treatment of the student pursuant to this authorization, the school is immune from all liability for damages or amounts incurred, including the cost of emergency care or transportation resulting from the examination or treatment of the identified student.

During the school year, opportunities arise to provide positive information and publicity about our programs and events to the general public or specific audiences.

In some cases, we may receive requests from the news media or professional persons to interview, photograph, and/or film students for:

- News or non-profit publications
- Television or radio broadcasts
- Educational information and training
- Publications and brochures printed by the school
- Parent-teacher organizations

Permission is needed for your child to be the subject of any news media publicity or to be included in school publications. Your selection will be kept on file for future reference and will remain in effect unless revoked in writing by the parent/guardian.

(a) A school district or open enrollment charter school shall notify a student's parent in writing at the beginning of the school year that if the student is absent from school on 10 or more days or parts of days within a six-month period in the same school year or on three or more days or parts of days within a four-week period:

(1) the student's parent is subject to prosecution under Section 25.093; and
(2) the student is subject to prosecution under Section 25.094 or to referral to a juvenile court in a county with a population of less than 100,000 for conduct that violates that section.

(b) A school district shall notify a student's parent if the student has been absent from school, without excuse under Section 25.087, on three days or parts of days within a four week period. The notice must:

(1) inform the parent that

(A) it is the parent's duty to monitor the student's school attendance and require the student to attend school; and
(B) the parent is subject to prosecution under Section 25.093; and

(2) request a conference between school officials and the parent to discuss absences.

(c) The fact that a parent did not receive a notice under Subjection (a) or (b) does not create a defense to prosecution under Section 25.093 or 25.094.
(d) In this section, 'parent' includes a person standing in parental relation.

What is a compact? This is a voluntary agreement between the school and the parents of the child at that school.

A compact outlines how parents, staff, and students are encouraged to share the responsibility for improved student achievement in meeting academic and non-academic goals.

Student Responsibilities:

• Maintain 90% attendance or better;
• Respect staff, peers, parents, visitors, and property;
• Work while you are at school and become a positive influence on those around you;
• Be a rule follower and trustworthy, and
• Know and be familiar with the student handbook.

Staff Responsibilities:

• Encourage students and expect appropriate student behavior;
• Teach students at appropriate levels using varying strategies;
• Expect student achievement and monitor student progress;
• Communicate with parents throughout the year;
• Communicate with students; and
• Create a positive learning environment.

Parent Responsibilities:

• Contact the school by phone in case of a student's absence or tardy;
• Contact the principal or teacher when there are concerns;
• Be involved in the student's educational process;
• Attend parent conferences and meetings;
• Communicate with your student; and
• Volunteer at school as appropriate.

Asbestos Notification: The Texas Department of Health has recommended that the charter school notify all parties involved that there is non-friable asbestos on the premises. If you have questions or concerns, please contact the principal.

Field Trip Travel Release: I grant consent for my student to participate in teacher and/or superintendent approved field trips. It is my understanding that the school will advise me by written or verbal notification of the nature,
date, and time of each field trip activity in sufficient time to enable me to communicate any withdrawal of consent for the specific trip or activity.
